Most regenerative treatments in aesthetics work by creating controlled injury that triggers a healing response. The inflammation from this injury is what drives the regeneration but also limits how aggressively the treatment can be applied and who can tolerate it. That Drives Regeneration
Adenosine A2A receptors are present on fibroblasts, endothelial cells, macrophages, and other tissue cells. When activated by PDRN or by naturally occurring adenosine, these receptors trigger a pro-regenerative and anti-inflammatory signaling cascade. In fibroblasts, A2A activation upregulates collagen synthesis genes, increases cell proliferation rate, and promotes migration to areas of tissue damage. In endothelial cells, it promotes angiogenesis, improving local tissue perfusion.
The anti-inflammatory effect of A2A activation comes from its ability to suppress the production of pro-inflammatory cytokines including TNF-alpha, IL-6, and IL-1beta while promoting the production of anti-inflammatory mediators. This shifts the tissue environment from pro-inflammatory to pro-regenerative, reducing the chronic low-grade inflammation that impairs tissue quality in aging and damaged skin and creating an environment more conducive to organized collagen deposition.
PDRN's polynucleotide chains are taken up by cells through endocytosis and processed in the lysosome, releasing their nucleotide components for use in cellular DNA repair and RNA synthesis. In UV-damaged fibroblasts with impaired function due to accumulated DNA damage, this exogenous nucleotide source supports the repair of damage that would otherwise persist and continue impairing collagen production. The combination of A2A receptor activation and nucleotide-level cellular support is what distinguishes PDRN from conventional growth factor treatments.

Experience
PDRN injections are administered with very fine needles using a grid-pattern micro-injection technique similar to skin booster treatments. Topical numbing cream is applied before treatment to ensure comfort. The injections are placed at intradermal depth across the treatment area, with the grid spacing calibrated to ensure even distribution of PDRN through the target tissue. The treatment takes 20 to 40 minutes for a full face or scalp.
Post-treatment effects are minimal compared to injury-based regenerative treatments. Small papules at injection sites resolve within two to four hours. Mild redness for 24 hours and occasional bruising are the most common post-treatment findings. Most patients return to normal activities immediately.
